Patient Online Access Request

Named accountable GP

From 1st April all practices are required under the 2015-16 General Medical Services Contract to allocate a named accountable GP to all practice patients, including children.

The named accountable GP will take the lead responsibility for the co-ordination of services required.

Any member of the practice team will be able to confirm the name of your accountable GP. This does not prevent you from seeing any doctor or nurse of your choice!

If a patient expresses a preference as to which GP they are assigned to, the practice will make reasonable efforts to accommodate this request.

IMPORTANT NOTICE!

Can all patients please be aware that ONE booked appointment is only ten minutes long.

This means that only ONE patient will be seen, and only ONE problem dealt with.

This will help everybody to be seen as close to their allotted appointment time as possible.

Clinics and Services

We offer a range of general medical care for all ages. For an appointment or further details, please call us on 0844 815 1100 or speak to one of our receptionists.

Antenatal / Maternity Care
If you become pregnant, please book a routine appointment with the doctor, who will refer you to the hospital of your choice for your maternity care. You will then be seen regularly throughout your pregnancy by the midwife and/or doctor and at the local hospital.

Your immediate postnatal care will be provided by the midwife team, with your postnatal check being provided by the practice team around 6 to 8 weeks after the birth.

Child Health & Immunisations

All new babies are invited for regular check-ups (at 8 weeks and 3.5 years) and immunisations from 8 weeks onwards. Appointments can be arranged through reception. PLEASE NOTIFY RECEPTION ABOUT THE REASON FOR THE APPOINTMENT, AS BABY CHECKS DONE BY THE GP REQUIRE ADDITIONAL TIME AND IMMUNISATIONS ARE DONE IN SPECIFIC CLINICS.

Contraceptive Services / Family Planning
Patients requiring contraception will need to book an appointment with the nurse who will discuss all the options available and give general health advice. For women taking the contraceptive pill a regular blood pressure check is required every six months. It is therefore essential to book an appointment with the nurse to receive the prescription. Emergency contraception is available (up to 72 hours after intercourse). Please tell reception if this is the reason for your appointment, so you can be seen in time.

Counselling
Patients requiring counselling will need to book an appointment with the GP to be referred to a member of the local Counselling Team.

Big_White_Wall_Logo

www.bigwhitewall.com  is a safe, anonymous service for people experiencing emotional or psychological distress provided entirely online. Professionally staffed 24/7 it offers a wide range of services for improving mental wellbeing including  tests, peer support, individual and group therapies, articles, tips and creative self expression.

Cryotherapy
Cryotherapy (freezing of warts, moles, skin tags etc with liquid nitrogen) can be arranged at our weekly clinic. Please discuss this with the doctor first before arranging an appointment, unless it is a follow-up.

Diabetes, Asthma, Heart Disease/Hypertension, Obstructive Airways Disease
These clinics offer advice and annual health check-ups to patients diagnosed with diabetes, asthma, chronic obstructive airways disease, hypertension or heart disease. PLEASE LET RECEPTION KNOW THAT YOU REQUIRE A CHRONIC DISEASE REVIEW APPOINTMENT AS THESE ARE LONGER THAN ROUTINE APPOINTMENTS.

Dressings / Suture Removal
If you have had surgery at the hospital or a local clinic and require a post-operative wound check or removal of sutures, you can book an appointment with our nurse in the dressing clinic. If the wound requires longer term attention we will refer you to the district nursing team. PLEASE ALWAYS INFORM RECEPTION THAT YOU REQUIRE A DRESSING APPOINTMENT, AS WE ALLOCATE SPECIFIC CLINICS FOR WOUND CARE TO REDUCE THE RISK OF INFECTION.

Minor Injury Care
If you have had a minor accident and wish to consult a GP we can see you in surgery unless you think you require an x-ray because of risk of fracture or you are bleeding heavily. In these circumstances you should go to A&E. For all other minor injuries you can book an appointment at the surgery.

Pregnancy
Please book an appointment with the nurse or GP who can organise your referral to the Midwife team. Should you wish to terminate the pregnancy, please discuss this with one of our GP's, who will be able to refer you.

Cervical Smears
All women registered with the practice, aged between 25 and 65 years of age are offered regular cervical screening. This important test can identify abnormal cervical cells in their very early, pre-cancerous stages and treatment can be arranged.
Travel immunisations
If you are travelling overseas we can arrange for you to receive your travel vaccinations – with the exception of yellow fever & Japanese encephalitis - at the surgery. As some vaccines need to be given several weeks before you travel, please make your appointment at least 4-6 weeks before your departure.  Although we do not charge for travel vaccinations, the NHS does not fund hepatitis B and malaria prophylaxis. Medication for Malaria prevention can be prescribed privately by the practice and purchased at the pharmacy of your choice.

For up-to-date information about what vaccines you require for your specific trip you can go to our travel information page (link on the right hand side of this page) or log on to the FIT FOR TRAVEL website. This site is provided to the public by the NHS Scotland.

Should you require vaccinations for cholera, rabies or tick borne encephalitis, you may need to collect a prescription prior to your appointment.

Travel immunisations are also available to patients not registered with this practice for a charge. Charges can be viewed here.
