YOU CAN NOW REQUEST FOR YOUR PRESCRIPTION TO BE SENT ELECTRONICALLY TO THE PHARMACY OF YOUR CHOICE. YOU WILL NO LONGER GET A PAPER PRESCRIPTION, BUT CAN GO DIRECTLY TO THE PHARMACY TO COLLECT YOUR PRESCRIBED ITEMS. PLEASE CALL RECEPTION TO LET US KNOW WHICH PHARMACY YOU WOULD LIKE TO USE OR COMPLETE AND RETURN THE ELECTRONIC PRESCRIPTION SERVICE NOMINATION FORM TO USE THIS SERVICE.

YOU CAN NOW BOOK & CANCEL APPOINTMENTS ONLINE

In order to use this service, you will need to register at reception. You will be given a unique ID and password to log into our 'SystmOnline' website where you can also order repeat prescriptions and view your medical record.

You will need to complete a registration form which you can complete online, print and bring with you to the surgery together with 1. Photo ID AND 2. Proof of residence (bank statement, utility bill or council tax bill).

Named accountable GP

From 1st April all practices are required under the 2015-16 General Medical Services Contract to allocate a named accountable GP to all practice patients, including children.

The named accountable GP will take the lead responsibility for the co-ordination of services required.

Any member of the practice team will be able to confirm the name of your accountable GP. This does not prevent you from seeing any doctor or nurse of your choice!

If a patient expresses a preference as to which GP they are assigned to, the practice will make reasonable efforts to accommodate this request.

Can all patients please be aware that ONE booked appointment is only ten minutes long.

 

This means that only ONE patient will be seen, and only ONE problem dealt with.

 

This will help everybody to be seen as close to their allotted appointment time as possible.

 

Patient Registration

Registration with the practice

We are always happy to accept new patients who live in our practice area (all addresses in the postcode districts: IG3, IG11, RM8, RM9 & RM10) to join our practice. Everyone who is a resident in the UK (living in the UK for more than 6 months) is entitled to register with a GP.

If you wish to register you will need the following documents ORIGINALS - NO PHOTOCOPIES:

  1. Photo ID (passport, driving license)
  2. Utility or council tax bill verifying your address (Water, Gas, Electricity, Council tax, Electoral Register Notification)
  3. NHS Medical Card (If you do not have your medical card, complete the below Registration Form without your NHS number)
  4. Please transfer your NHS number from your medical card and complete and print the Highgrove Surgery Registration Form - link below -and bring it to the surgery. PLEASE ENSURE THAT ALL DETAILS ARE COMPLETED TO AVOID DELAYS WITH REGISTRATION.
  5. Complete either an Adult Registration Form (for patients over the age of 14) or a Child Registration Form for every person who wishes to register.

Please bring these documents to the surgery. We will check the first two documents and record your Photo ID number. The Medical Card and registration forms are required to confirm your details with the NHS record service and setup your entry on our database.

You will then be given an appointment with our Health Care Assistant for a registration medical.


Please note that these forms can be completed by typing into the fields and using the 'tab' key or the mouse to move from field to field.

Please complete before printing to reduce the number of illegible forms being submitted.
