YOU CAN NOW REQUEST FOR YOUR PRESCRIPTION TO BE SENT ELECTRONICALLY TO THE PHARMACY OF YOUR CHOICE. YOU WILL NO LONGER GET A PAPER PRESCRIPTION, BUT CAN GO DIRECTLY TO THE PHARMACY TO COLLECT YOUR PRESCRIBED ITEMS. PLEASE CALL RECEPTION TO LET US KNOW WHICH PHARMACY YOU WOULD LIKE TO USE OR COMPLETE AND RETURN THE ELECTRONIC PRESCRIPTION SERVICE NOMINATION FORM TO USE THIS SERVICE.

YOU CAN NOW BOOK & CANCEL APPOINTMENTS ONLINE

In order to use this service, you will need to register at reception. You will be given a unique ID and password to log into our 'SystmOnline' website where you can also order repeat prescriptions and view your medical record.

You will need to complete a registration form which you can complete online, print and bring with you to the surgery together with 1. Photo ID AND 2. Proof of residence (bank statement, utility bill or council tax bill).

Named accountable GP

From 1st April all practices are required under the 2015-16 General Medical Services Contract to allocate a named accountable GP to all practice patients, including children.

The named accountable GP will take the lead responsibility for the co-ordination of services required.

Any member of the practice team will be able to confirm the name of your accountable GP. This does not prevent you from seeing any doctor or nurse of your choice!

If a patient expresses a preference as to which GP they are assigned to, the practice will make reasonable efforts to accommodate this request.

Can all patients please be aware that ONE booked appointment is only ten minutes long.

 

This means that only ONE patient will be seen, and only ONE problem dealt with.

 

This will help everybody to be seen as close to their allotted appointment time as possible.

WE ARE NOW PROVIDING A ONE STOP WEBSITE WITH A SECURE LINK TO OUR DATABASE. THIS WEBSITE WILL ALLOW YOU - ONCE REGISTERED - TO ORDER REPEAT MEDICATION, BOOK AND CANCEL APPOINTMENTS AND VIEW YOUR MEDICAL RECORD IF YOU WISH.

YOU CAN COMPLETE THE Online Access Registration Form  AND HAND IT TO RECEPTION TOGETHER WITH

  1. PHOTO ID AND

  2. A PROOF OF RESIDENCE DOCUMENT (Bank statement, utility bill or council tax bill)

YOU CAN THEN FOLLOW THE LINK ABOVE TO LOG ON

Please do not wait until you are out of medication before requesting a repeat prescription!

We do not accept requests for repeat prescriptions by telephone. This is to avoid errors with your medication request and to avoid prolonged engagement of the telephone lines.

If you do not wish to order your repeat prescription online via the link above, please use the right hand side of your previous prescription (white background) and tick all the items you require. Alternatively, you can write down your request but please make sure that you include the drug name and form (eg. Lasix capsules), strength (eg. 40mg), dosage (how many a day) and the amount required as well as your full name and date of birth. For your convenience we can provide a form for you to fill in (available at reception). Please keep a copy of this form for future requests

You can hand your request in personally, fax it to us, send it by post or order online following the link above.   If you include a stamped, self addressed envelope we will post it back to you.

If you are registered for prescription requests you will also be able to book and cancel appointments online.

Please allow 2 full working days (48 hours) for your prescription to be ready plus additional time for post.
